Silent Hill 2's Pyramid Head design came to life from a painting in the game, not James's mind. This information came from a recent comment by art director Masahiro Ito.

"To use [Pyramid Head] in so many titles makes [Pyramid Head] cheaper," Ito said in a now deleted post. Konami partnered with developer Bloober Team to recreate the game where Pyramid Head debuted.

Between Pyramid Head's unsettling design and symbolic function in Silent Hill 2's narrative, he remains one of gaming's greatest monsters.
